What is bone grafting?
Bone grafting is a surgical procedure that rebuilds or augments the bone in the jaw to create a stable foundation for dental implants.
When a tooth is lost, the bone that once surrounded and supported the root begins to resorb, a natural process where the body gradually breaks down bone that is no longer being stimulated. The longer a tooth has been missing, the more bone volume is typically lost. In some cases this bone loss is significant enough that there is insufficient volume remaining to place a dental implant safely and successfully without preparatory work first.
Bone grafting addresses this by adding bone material to the deficient area, encouraging the body to regenerate new bone tissue over a healing period. Once the graft has integrated and sufficient bone volume has been restored, implant placement can proceed.
It is one of the most commonly needed preparatory procedures in implant dentistry and one of the most important. Without sufficient bone, an implant has no stable foundation to integrate with. With the right bone volume in place, the implant can be placed with confidence and a high likelihood of long-term success.

Types of bone grafting we offer
Socket Preservation Graft
Carried out at the time of tooth extraction to preserve bone volume at the site immediately after the tooth is removed. By placing graft material into the socket before it heals, we significantly reduce the amount of bone that would otherwise resorb during the healing period, keeping the site in the best possible condition for implant placement later.

Ridge Augmentation
Where bone volume has already been lost following tooth extraction or long-term tooth absence, a ridge augmentation graft rebuilds the height and width of the bone ridge to a level sufficient for implant placement. The graft material is placed at the deficient site and covered with a membrane to guide bone regeneration over the healing period.

Sinus Lift
The maxillary sinuses, the air-filled cavities above the upper back teeth, can expand downward into the space left by missing upper teeth, leaving insufficient bone height for implant placement in that area. A sinus lift procedure adds bone graft material beneath the sinus floor, raising it and creating the bone height needed for upper arch implants.

Block Bone Graft
For patients with more significant bone deficiencies, a block of bone may be taken from another area of the jaw and secured at the recipient site to rebuild a larger volume of bone. Used where the deficiency is too large to be addressed with particulate graft material alone.

Do I need bone grafting before implants?
Not every implant patient needs bone grafting. Many patients have sufficient bone volume to proceed directly to implant placement without any preparatory work. However, bone loss is extremely common, particularly in patients who have had missing teeth for some time, those who have worn dentures for years or those who have had significant gum disease.
You may need bone grafting if you:
- Have had a tooth missing for a year or more
- Have been told you have insufficient bone for implants
- Have experienced significant gum disease that has affected the bone
- Have worn a full or partial denture for an extended period
- Are planning implant treatment in the upper back jaw where the sinuses may be a factor
You may not need bone grafting if you:
- Are having an implant placed shortly after extraction
- Have good bone volume confirmed on x-ray or CBCT scan
- Are replacing a tooth that has been missing for a relatively short period

What does bone grafting treatment involve?
Free consultation and assessment
Your first appointment is free of charge. Dr Nikhil will carry out a full clinical assessment, take x-rays and in most cases a CBCT scan in-house to accurately assess the extent of bone loss and determine the most appropriate grafting approach. You will leave with a clear written treatment plan and full cost breakdown with no obligation to proceed.
Treatment planning
The CBCT scan gives a precise three-dimensional picture of your jaw structure, bone density and the proximity of anatomical structures such as the sinuses and nerve channels. This level of detail allows Dr Nikhil to plan the grafting procedure and subsequent implant placement with accuracy and confidence.
Bone grafting procedure
The grafting procedure is carried out under local anaesthetic. The graft material, which may be synthetic, donor or taken from another part of your own jaw depending on the type of graft required, is placed at the deficient site and secured in position. A protective membrane is typically placed over the graft to guide bone regeneration. Sedation is available if preferred.
Healing period
The graft is left to integrate with the surrounding bone over a healing period that typically ranges from three to six months depending on the type and extent of the graft. During this time the body generates new bone tissue around the graft material, gradually restoring volume and density to the site.
Assessment and implant planning
Once the healing period is complete, a further CBCT scan or x-ray is taken to confirm that sufficient bone volume has been achieved. If the graft has integrated successfully, as is the case in the vast majority of procedures, implant placement planning can begin.
Implant placement
With the bone volume now sufficient to support an implant, the implant placement procedure proceeds as standard. The entire implant treatment plan from this point follows the same pathway as for any implant patient at Brunner Court.
